What it is
11,828 county estimates from CDC PLACES (release 2025, observation year 2023): four age-adjusted measures for adults — diagnosed diabetes, obesity, high blood pressure and lack of health insurance (ages 18–64). Each row is one county and measure with its estimate and 95% confidence interval, labelled by state, category and measure.
How to use it
Compare needs across states and measures; always read the confidence interval. Estimates are modelled, not counts: don't multiply by population.
Source and changes
Centers for Disease Control and Prevention, PLACES (data.cdc.gov/d/swc5-untb), available at no charge. We kept state, category, measure and the estimate with its interval; county names, FIPS codes, coordinates and population counts were removed for this release. Use does not imply endorsement by CDC, ATSDR, HHS or the U.S. Government.
Limitations
Kentucky and Pennsylvania lack 2023-based estimates in this release; small counties have wide intervals.
